Fixes:

Co-op setup button no longer disappears after playing a level
Failure song plays when dying in Co-op mode instead of Victory song
Shroomian Hero Charm status icon no longer appears on small health bars
Player 2 now has own spawn and death animations
Keyboard and Controller navigation to Co-op button from Options menu works now
Accomplishment pop-up now properly displays Trinket Slot rewards
